Grafting in The Sims 4 can be a little confusing at first, but the process isn't overly difficult. After reaching Level 5 in the Gardening skill, Sims can interact with a plant and cut a sample from it by choosing the "Take Cutting" option. Once the Sim has taken the sample, they can interact with a different plant and select the "Graft" option to graft the sample onto it.

It's impossible to graft certain plants with others, so players will need to search the world until they find the right combinations.

Then, players will have a chance to choose what sample they wish to graft onto the plant. Once they have chosen the sample they want, they can select it to immediately graft it onto the plant they are interacting with.

Hovering the cursor over a spliced plant will reveal what it will grow, but players might need to wait a while if they are growing something particularly rare, as common plants tend to take priority. So long as players are patient and tend to their plants daily, the flower or fruit they seek will eventually grow and can be harvested. Players can then plant what they harvest to grow new plants.
