This activity is a lovingly nostalgic nod to the special “Puff-Puff” service featured in most Dragon Quest games, a series which Like a Dragon's protagonist Ichiban Kasuga credits as an inspiration for his own hero’s journey. In these games, players could pay a villager for this cryptic, sexually-charged service, typically resulting in a humorous cutscene of the aftermath.

Where to Find All Honk-Honk NPCs
Like a Dragon: Infinite Wealth features a total of 14 Honk-Honk NPCs hidden throughout Honolulu, Isezaki Ichinjo, and Kamurocho. Kasuga or Kiryu, depending on chapter and location, can speak with these NPCs and are presented with the opportunity to pay money to “Get Honked.” What actually happens during said honking isn’t revealed or explained, perhaps for the better.
Honk-Honk NPCs may not always be present at their designated locations. If you don't find them during your initial visit, consider returning at another time or during a different part of the day.
The outcome of each is a one-time boost to either Kasuga's Personality or Kiryu's Awakening stats. Below, the locations of each Honk-Honk NPC in Like a Dragon: Infinite Wealth have been outlined. The bulk of Kasuga's adventures in Like a Dragon: Infinite Wealth unfolds in Honolulu, where he joins forces with the renowned Kiryu Kazuma to tackle the challenges laid out in the main storyline. All Honk-Honk Locations in Isezaki Ichinjo and Kamurocho, Japan
As Kiryu spends the majority of his time in Isezaki Ichinjo, he's going to be the recipient of its fine honking services and, frankly, he could probably use the relief. Whatever that is. Receiving a honking will increase Kiryu's exclusive Tech, Body, and Soul Awakening stats. He will automatically unlock the services of the Honk-Honk NPCs during Chapter 10 of Like a Dragon: Infinite Wealth, but he will not be able to access Kamurocho's Honk-Honk Princess until a later chapter.
