Lisa Trevor is one of the most tragic figures in Resident Evil, being experimented upon by the horrid Umbrella Corporation. These tests turned her into a grotesque and powerful abomination who can be quite scary to behold.
Players have to run as fast as possible whenever they encounter this mass of flesh in the first game time and time again. Despite turning into a monster, Lisa still holds the memories of her past and exhibits rather odd behavior when around anyone or anything that triggers her emotions.
8 Hannah Washington – Until Dawn
Hannah is a character in Until Dawn who is forced to become a Wendigo after getting lost in the woods and cannibalizing her friend. Her alternate form is truly scary, with the protagonists of Until Dawn being tormented by her presence time and time again.
Hannah's story is one of tragedy, continuing a theme of people being forced against their will to become horrifying monstrosities with little to no memories of their past lives. Her Wendigo form scares players time and time again, making each and every encounter with her a nail-biting affair.
7 Lady Dimitrescu – Resident Evil Village
Lady Dimitrescu from Resident Evil Village is a towering, vampire-like woman who took over the internet by storm with her unique appearance. Her imposing figure and scintillating personality make her a standout character in the game's cast of memorable villains.
Most people would argue that she isn't even all that scary... but that's easy to say when players haven't experienced how it feels to be chased by this massive creature. The fact that she isn't even a huge part of the main plot is quite surprising, especially since many people thought that she was the main antagonist of Resident Evil Village!
6 Kirie Himuro – Fatal Frame
Fatal Frame is a series filled to the brim with female villains across the board. Choosing just one of the many ghosts that are present in these titles can be quite a challenge, but props should be given to the horrifying Kirie Himuro, who is present in the first game.
A shrine maiden who was supposed to maintain her purity, Kirie developed feelings for a person right before her sacrifice. The priests found out about this and strove to kill this man to prevent any complications, but Kirie learned about the murder of her love moments before her sacrifice, leading to a flood of ghosts appearing after her demise as she turned into a vengeful spirit.
5 Marguerite Baker – Resident Evil 7: Biohazard
Resident Evil 7: Biohazard features the Baker family as the antagonists of this tale. Each member of the family fell victim to Eveline's mold, and Marguerite was no exception to the effects of this mind-altering substance.
Any semblance of her old identity was lost, leading to Marguerite being nothing more than a berserk villain hell-bent on killing Ethan Winters, the intruder in her house. Every encounter with her is a chilling affair, and players will be glad to finally take her out after being tormented by this member of the Baker family.
4 The Witch – Left 4 Dead
The Witch is a mysterious and dangerous creature in Left 4 Dead that can kill players in a single hit, making her one of the most feared enemies in the entire game. Her behavior is extremely unpredictable, with her cries and screams invoking a sense of dread as players attempt to sneak past her without raising the alarm.
Most players may feel sorry for this creature as she sits in a corner and cries. However, this sympathy will soon be replaced with a sense of pure and abject terror if they make the mistake of letting it know about their presence.
3 Alma Wade – F.E.A.R.
Alma Wade is one of the most iconic characters in gaming history, with this terrifying and powerful psychic entity being created by the vice of humans who wanted to harness her power for their own nefarious purposes. She is truly disturbing in every encounter and seeks revenge against humans for forcing her to endure numerous experiments and inhumane torture.
Encountering her is a tense moment in any F.E.A.R. game, making her one of the most memorable female villains in any video game, let alone the horror genre. She may have a tragic origin story...but that's the last thing players will remember when they're being actively hunted by this nightmarish antagonist.
2 SHODAN – System Shock
SHODAN is an AI that was hacked early on in the first System Shock game, causing her to go berserk as she terrorized Citadel Station and either killed or mutated its inhabitants. The hacker who sowed the seed of chaos that caused her to go rogue in the first place embarks on an uneasy adventure as he manages to purge SHODAN by the end of the first game.
However, her attempts to dominate humanity didn't stop here, making a memorable appearance in the second game in a plot twist that many people didn't see coming. Her erratic manner of speaking and a chilling god complex make her a riveting antagonist, and while she may not be a woman in the traditional sense, no one would really question her inclusion on this list.

P.T. is considered by many to be one of the greatest interactive horror experiences of all time. The puzzles in this game were so obtuse that this free game fostered a unique community of its own as players interacted online to figure out the solutions to P.T.'s many riddles and mind-teasers.
Unfortunately, players soon realized that they weren't alone in this looping hallway. The spirit of Lisa haunted players every step of the way, and this nightmarish entity's mere presence was so effective that players were constantly in a state of unease as they went through this chilling playable teaser.
