Keeping this in mind, this guide will cover everything there is to know about playing as a Ghoul in Fallout 4, along with other useful information.

It is worth noting that you cannot become a Ghoul in vanilla Fallout 4, but there is a perk that will allow you to easily survive the radioactive world around you. The Ghoulish perk can be acquired from the Perks and S.P.E.C.I.A.L. skill tree.

To get the Ghoulish perk, you need to have Level 9 Endurance. The best thing about this perk is that radiation will heal you and, if you max out this perk, radiation will become a non-issue for you. If this workaround does not cut it for you because you want your avatar to look like a Ghoul, then you will need to install mods.

You can install mods via the in-game mod browser, and there are a couple of mods that will allow you to live out your fantasies by playing as a Ghoul in Fallout 4. For instance, Cebius' Revamped Playable Ghoul mod is worth checking out.

To install the mod, here is what you need to do:

  1. Start the game or head back to the main menu.
  2. Select Mods.
  3. Use Search to find the mod of your choice.
  4. Install the mod.

Once the mod is enabled, you can continue your game as a Ghoul, but you will likely want to start over from scratch.

By combining the mod with the Ghoulish perk, you can role-play as a Ghoul. Unfortunately, nobody will react to your appearance, but this could be fixed with later updates to the mod. Now that you know how to play as a Ghoul in Fallout 4, make the most of this information and play the game as you desire, which is what Bethesda games excel at.
