One of the most malicious and dangerous is Malus Thorm, and slaying him is also essential to breaking the Shadow-curse and recruiting Halsin. The protagonist can find him and his minions, the Sisters of Mercy, in the local hospital and medical school.

As it is with many of the boss fights in BG3, there are a variety of ways to win. If Tav plays the dialogue options in a certain way, they'll have access to three unique weapons that can't be found anywhere else in the game.

Where To Find The House Of Healing

Reithwin Town is in the western part of the Shadow-cursed Lands and is easily accessible via a few shabby bridges. Players who choose to join the Drider and his party from Moonrise Towers will go there directly, while those who side with the Harpers head to Last Light Inn first.

  • From the Reithwin Town Waypoint: Walk west along the main road that leads through the center of Reithwin. The main doors of the House of Healing are the big building on the north side of the road, across from The Waning Moon Tavern. Sister Lidwin is at the reception desk and a Deception check is needed to get past her.
  • From the Moonrise Towers Waypoint: Walk north, and you can either take a left and stay on the main road and use the main doors on the north side of the street, or continue north and turn left into the graveyard, which also leads to the House of Healing side and back doors. It's also possible to climb a ladder and get in through a hole in the roof.

Confronting Malus Thorm

  • Each Sister drops a unique weapon - but only under certain circumstances.

Each of the Thorms, except Ketheric, can be talked down to surrender or suicide rather than fight. The options Tav can use to do this depend on their skills and stats in certain areas. Rogues, Bards, and other classes with a typically high Charisma have a better chance at succeeding in these rolls.

There are three ways to obtain the Sisters' weapons.

  • The protagonist has to pass an Investigation or Insight check, sometimes class-related, to recognize that the instruments the Sisters are using are dull. An option will appear in the dialogue to order Malus to tell the sisters to practice the craft on themselves. Once they kill each other, you can take their weapons.
  • Another option is to skip the dialogue and simply fight everyone. The weapons in question will still drop from the Sisters' corpses.
  • Should Malus kill himself first, and the Sisters live, it's possible to pickpocket the weapons instead.

The Sisters' Unique Weapons

It's not just the theme of these weapons that makes them fun, but the design and sometimes unique abilities provided. All three are of Uncommon currency and there's a wide enough variety that can fit any race or class.

Trepan

The Trepan's actual use in medicine is to bore holes in bone and cartilage. Only this weapon offers a special ability, Trepanation, other than the usual melee skills of Flourish and Piercing Strike.

  • Shortsword: A light finesse weapon that various classes can use, and those that have a high Dexterity can use the higher modifier instead.
  • Stats And Abilities: Piercing damage, 1d6 damage + Strength or Dexterity modifier
  • Trepanation: Bore a hole through the enemy's skull and possibly give them the Prone condition.

Bonesaw

  • Longsword: A martial melee weapon, the Bonesaw is limited to Fighters, Rangers, and other classes that have proficiency with heavier, more complex armaments and use their Strength modifier for damage as opposed to Dexterity.
  • Stats And Abilities: Slashing damage, 1d8 + Strength modifier for dual-handed usage, 1d10 + Strength modifier for characters using it alone or with a shield

Syringe

  • Dagger: A light, simple finesse weapon that can also be thrown, the Syringe is accessible to virtually every class, even spellcasters.
  • Stats And Abilities: Piercing damage, 1d4 + Strength or Dexterity modifier
