After three years since the release of the first chapter in the series, A Plague Tale: Requiem explores the consequences of the powers of the Prima Macula on the original Carrier, which is a juxtaposition to the first title, as it instead focused on the villainous Vitalis Benevent. The game reprises the same stealth elements of A Plague Tale: Innocence and players can once more control Amicia de Rune as she navigates her role of Protector in a new capacity. Some characters from the first game make a comeback, including Amicia and Hugo's mother, Beatrice, and Lucas, the young alchemist in training who stayed with the de Rune family.
A Plague Tale: Requiem missed the opportunity to bring back Melie, who played a major role in the first chapter of the series and ended up being someone the players grew fond of. With twists and turns that are possibly even darker than in A Plague Tale: Innocence, the sequel does a great job of providing new and returning characters with some form of closure. This is not necessarily the end of the saga, but it's likely the final chapter of the de Rune family's story, especially considering all the characters that end up dying.

Vaudin
Vaudin is an alchemist that appears in the first few chapters of A Plague Tale: Requiem, whose role is to try and stop the effects of the Macula advancing on Hugo, who is seemingly losing control of the rats and his powers. In the Protector's Duty chapter, he suggests that Hugo is transported to Marseilles, to the Order's headquarters, where he can be monitored. However, Vaudin's treatments make Hugo have a seizure, during which the rats destroy the city. When Amicia finally gets to her brother to calm him down, the rats make part of their building collapse, killing Vaudin.
Joseph
Joseph is a sailor who is in charge of taking the de Rune family to Marseilles via boat, and he debuts in the Protector's Duty chapter. Unfortunately, he doesn't live long enough to make much of an impression, as he shortly after is killed by an archer with an arrow through the neck while sailing to Marseilles.
Milo
Milo is a slaver who previously worked for Countess Emilie de Arles from La Cuna island but then moved to another part of the island where he performed sacrificial offerings to the Child of Embers - Hugo. He does so because he wants to rule the island by pleasing Hugo, who could give him control of the rats. However, in a confrontation with Hugo, Amicia, and Sophia, Milo is killed by the latter with a knife through the chest.
Beatrice de Rune
Amicia and Hugo's mother was thought to be dead in the prologue of A Plague Tale: Innocence, but she was later found by her children and rescued. In the sequel, her role as an alchemist is made clearer, and she tries to take her children to Marseilles alongside Joseph. Beatrice is captured by the Countess, and Amicia's efforts to save her are in vain as she is defeated by the Count, Victor de Arles. The Countess performs a similar sacrificial offering as those of Milo, and she slits Beatrice's throat in front of Amicia.
Countess Emilie de Arles

Shortly after the Countess murders Beatrice, Hugo gets to the amphitheater of La Cuna, where his mother's corpse is tied to a pole, with the Countess clumsily trying to hide the knife she used to kill her. This is because Emilie and Victor de Arles want to make Hugo their son as the Child of Embers story foretold, but the plan rapidly goes up in smoke when the boy realizes what happened. At that point, Hugo conjures the rats to take out the Count's men and then the Countess, who is devoured by the plague carriers.
Arnaud and Count Victor de Arles
Arnaud is initially opposing Amicia on her journey, as he is a knight to Count Victor de Arles and captures her and Lucas at one point. However, he becomes an ally when he notices Hugo's powers, which he believes is the only way out alive. He aids the de Rune family on multiple occasions throughout A Plague Tale: Requiem, but is eventually struck dead by the Count himself, who drives a sword through the knight's abdomen.
However, Arnaud grabs the sword's blade and takes it deeper into his guts to get closer to the Count, and then removes his helmet. Victor the Arles then pulls the sword out and moves toward Amicia to kill her too, but she manages to hit him between the eyes with her sling, instantly killing him. She then runs to Armaud, who tells her it's the right time for him to die, which he does moments later, reinforcing the same theme from A Plague Tale: Innocence.
Hugo de Rune
In perhaps the darkest twist in the entirety of A Plague Tale: Requiem, Hugo gives in to the Macula after believing Amicia and those he loved were all dead. He does so because he wants to avenge them, a sentiment he repeatedly shows throughout the game, but this time there is no going back. After Amicia finally accepts that Hugo has to die, however hard that may be, she runs into Lucas, and together they silently march toward Hugo.
However, players have a choice here: either they kill Hugo as Amicia or do nothing, which changes A Plague Tale: Requiem's ending slightly. If players allow Amicia to do it, she then uses her sling to kill her brother. A year later, Amicia reunites with Sophia, and she mentions Lucas is studying alchemy somewhere. If players do nothing, Lucas then kills Hugo with his crossbow. In the end, Amicia then tells Sophia that Lucas can't be there with them because she still sees him do it in her head, showing resentment towards him.
